This Membership Agreement sets forth the terms and conditions of your participation in a Compass NeuroSpine, PLLC (“Compass NeuroSpine”) Pathway, including the terms and conditions on which Compass NeuroSpine will provide services to you. You therefore agree as follows:
Compass NeuroSpine provides direct, individualized, neurosurgical care for patients who need ongoing evaluation, decision-making assistance and follow-up through three (3) different pathways (each a “Pathway”) as set forth below.
Compass NeuroSpine does not bill Medicare or any other insurance company, and you agree to pay for Compass NeuroSpine’s services in the amount applicable to your selected Pathway. The pricing for each Pathway is all-inclusive for the services included in the applicable Pathway. Given the Pathways and all-inclusive pricing structure, you understand and acknowledge that neither Compass NeuroSpine nor you can bill Medicare or any other payor for the services provided under this Membership Agreement. Notably, Compass NeuroSpine’s physician has opted out of the Medicare program and does not participate with any other insurance company. If you are a Medicare beneficiary, please see Exhibit A which contains additional terms applicable to the services provided to you by Compass NeuroSpine.
Compass NeuroSpine Pathways
Pathway 1: NeuroSpine Point of Care (POC)
A single comprehensive neurosurgical evaluation and care plan.
Includes:
Full neurosurgical intake
Review of records and imaging
Clinical assessment
Treatment recommendations and next steps
Fee: $3,500
Note: If you enroll in Pathway 2 or Pathway 3 within 14 days of signing this Membership Agreement, $1,500 is credited toward your fee for Pathway 2 or Pathway 3, as applicable.
Pathway 2: NeuroSpine Comprehensive Care
Defined longitudinal neurosurgical care with clinical accountability.
Includes:
Ongoing neurosurgical oversight
Follow-up visits as clinically appropriate
Treatment planning and coordination
Clinical documentation and decision-making
Direct physician involvement throughout
Best suited for:
Patients requiring monitoring or staged decisions
Post-evaluation follow-up
Those seeking continuity rather than episodic visits
Fee: $5,000 | 12 weeks
Pathway 3: NeuroSpine Priority Care
Enhanced clinical care for complex or time-sensitive cases.
Includes everything in Comprehensive Care, plus:
Priority scheduling and access
Increased follow-up cadence
High-touch coordination
Availability during critical decision window.
Best suited for:
Complex surgical planning
Post-operative management
Situations where timing and nuance matter
Fee: $7,500 | 12 weeks
